Adult onset nesidioblastosis after gastric bypass surgery, a clinical and pathologic review

چکیده
Despite the many benefits of bariatric surgery in patients with morbid obesity, concerns have been raised over its association with severe hyperinsulinemic hypoglycemia and nesidioblastosis following Roux-en-Y gastric bypass surgery. Cases have been described with severe post-prandial hypoglycemia with neurological symptoms following surgery. Some of these cases are recalcitrant to medical treatment and may require partial pancreatectomy. Histological examination of the resected pancreas specimens shows features of adult nesidioblastosis characterized by hyperplasia of islets of Langerhans and beta cell hyperplasia and hypertrophy. Although the underlying mechanisms of gastric bypass-associated nesidioblastosis and hypoglycemia are still not fully understood, the exaggerated gut hormone response to altered nutrient flow in Roux-en-Y gastric bypass may be the cause of nesidioblastosis, hyperinsulinemia, hypoglycemia, and relevant neurological symptoms. We summarize the clinical and pathological features of adult nesidioblastosis following the Roux-en-Y gastric bypass procedure. The underlying mechanisms of post gastric bypass-associated hypoglycemia and nesidioblastosis are also discussed in this review.

Robotic Near-Total Pancreatectomy for Nesidioblastosis after Bariatric Surgery
Postprandial symptoms of neuroglycopenia after bariatric surgery may result as a consequence of endogenous hyperinsulinemic hypoglycemia (nesidioblastosis) not dumping syndrome. Pancreatectomy is an acceptable treatment for this condition. We present the video of a case of near-total distal robotic pancreactectomy for the treatment of nesidioblastosis after Roux-en-Y gastric bypass. Robotic pan...
متن کاملHypoglycemia After Gastric Bypass Surgery
Severe hypoglycemia characterized by neuroglycopenic symptoms is a recently described and relatively uncommon complication of gastric bypass surgery. It occurs several months to years after surgery and may be distinct from the more commonly encountered dumping syndrome that occurs early in the postoperative course and usually improves with time. Nesidioblastosis has been proposed as a possible ...

متن کاملHyperinsulinemic hypoglycemia with nesidioblastosis after gastric-bypass surgery.
We describe six patients (five women and one man; median age, 47 years; range, 39 to 54) with postprandial symptoms of neuroglycopenia owing to endogenous hyperinsulinemic hypoglycemia after Roux-en-Y gastric bypass surgery. Except for equivocal evidence in one patient, there was no radiologic evidence of insulinoma. Selective arterial calcium-stimulation tests, positive in each patient, were u...
